Please fill out a separate copy of the questions for each person you are registering):

• Which workshop(s) or class(es) you are registering for (please list class time and title)
• Your "Lead" or "Follow" status for classes:
• Will you be paying by PayPal or Credit Card (online), or by Cash, Check , or Money Order (in person, or by mail)?
